Ascendis Pharma A/S (ASND) has reported five-year Phase 2 results showing that TransCon PTH, also known as palopegteriparatide, continued to mimic the systemic activity of natural parathyroid hormone in adults with hypoparathyroidism. The long-term data from the PaTH Forward trial showed sustained efficacy and safety across multiple organ systems.
The study found that TransCon PTH helped normalize and stabilize serum calcium levels while improving kidney function, bone mineral density, and health-related quality of life measures. The therapy also allowed many patients to reduce or discontinue conventional treatments, including active vitamin D and high-dose calcium supplements.
At Week 266, 82% of patients achieved the multi-component endpoint of maintaining normal serum calcium without active vitamin D and using less than 600 mg per day of calcium. Nearly all participants became independent from vitamin D and therapeutic calcium, while 95% completed the full five years of treatment. Kidney function improvements were maintained, with mean eGFR increasing from baseline and offsetting the decline typically expected with age.
Patients also reported rapid and durable improvements in physical and cognitive symptoms, daily functioning, and overall quality of life, based on validated assessments including HPES and SF-36. Bone mineral density improved early in treatment and remained stable through the five-year follow-up.
TransCon PTH was generally well tolerated, with no new safety signals identified. Most adverse events were mild or moderate, and no treatment discontinuations were attributed to the drug. One patient developed transient, non-neutralizing anti-PTH antibodies, with no observed effect on efficacy or safety.
Presented at the European Congress of Endocrinology 2026, the findings support the potential of TransCon PTH as a new standard of care for chronic hypoparathyroidism by addressing the underlying hormone deficiency rather than focusing only on symptom management.
