The FDA has awarded breakthrough device designation to Aurenar’s V-Link system, a non-invasive nerve stimulation platform developed to address cerebral vasospasm — a dangerous and common complication following brain hemorrhage.
Vasospasm occurs when cerebral blood vessels constrict in the days after an aneurysm rupture, restricting blood flow and threatening secondary brain injury. A significant proportion of patients who survive the initial hemorrhage face this additional, often more severe neurological insult.
Historically, clinicians have had limited preventive options. The standard approach relies on an oral medication, with invasive interventions such as balloon angioplasty reserved for rescue scenarios.
Aurenar’s V-Link seeks to intervene earlier in the pathological cascade. The earbud-style device delivers mild electrical pulses to a nerve behind the ear, modulating the inflammatory response believed to underlie vessel spasms. Treatment consists of 20-minute sessions twice daily during ICU admission, designed to integrate seamlessly into existing workflows with minimal training required, according to CEO Eric Leuthardt.
In a 27-patient randomized, triple-blinded trial — where patients, clinicians, and outcome assessors were all masked — the device reduced moderate-to-severe vasospasm by over 40% without reported safety concerns. Leuthardt emphasized that the effect size reached statistical significance and aligns with known biological mechanisms of vagus nerve stimulation on inflammation.
While acknowledging the trial’s modest scale, Leuthardt stated the company is pursuing further evidence generation “with the appropriate humility.” The breakthrough designation is intended to accelerate the path toward larger pivotal trials, which will confirm whether the observed benefits persist at scale.
Near-term efforts remain focused on pivotal studies for the vasospasm indication, though Aurenar envisions the same anti-inflammatory mechanism eventually addressing other high-acuity ICU conditions through sequential indication expansion.
