Key Points
-
Intuitive Surgical operates the da Vinci surgical robot, with 11,710 systems in use globally.
-
Competitors Medtronic and Johnson & Johnson are launching their own surgical robot systems.
-
Intuitive Surgical’s primary revenue source lies in recurring service and instrument sales, not new robot sales.
Intuitive Surgical (NASDAQ: ISRG) has experienced significant stock volatility, currently trading 40% below its early 2025 peak with a P/E ratio of 42x. While this reflects elevated risk, the company’s long-term growth trajectory and market leadership position could justify continued investment consideration for growth-focused investors.
Intuitive Surgical isn’t alone anymore
Once the sole leader in surgical robotics, Intuitive Surgical established the da Vinci platform as the industry standard. Surgical robotics have since become widely adopted for minimally invasive procedures, but new entrants are challenging its dominance. In late 2025, Medtronic secured FDA approval for its Hugo robotic system, while Johnson & Johnson received clearance for its OTTAVA platform in mid-2026. Both companies have deep healthcare industry networks, positioning them as serious competitors.
However, regulatory approvals for surgical robots are procedure-specific, limiting immediate overlap with Intuitive Surgical’s existing portfolio. The company sold 468 da Vinci systems in Q2 2026 versus 395 in Q2 2025, suggesting sustained demand despite emerging competition.
Intuitive Surgical’s real flywheel isn’t selling robots
Though da Vinci system installations have grown 12% year-over-year (11,710 total globally), a more critical metric is the 16% increase in procedures performed using these systems. This growth highlights expanding utilization but underscores Intuitive Surgical’s true revenue driver: its services and accessory ecosystem.
The company derives approximately 75% of revenue from recurring sales of surgical instruments and services contracts, far exceeding revenue from new robot sales (25%). This annuity-like model creates predictable cash flow: as installed base grows, so does demand for consumables and maintenance. This structure provides Intuitive Surgical with durable competitive advantages, even as new players gain traction.
There’s room for more than one surgical robot
The global surgical robot market remains vast, with room for multiple platforms. Intuitive Surgical’s first-mover advantage and ecosystem lock-in give it critical scale, while newcomers face steep innovation hurdles. For instance, the da Vinci network includes trained surgeons and service centers that other platforms will need to replicate.
From a valuation perspective, the stock’s current P/E of 42x is notably lower than its five-year average of 68x. This suggests the market is pricing in near-term challenges without fully appreciating the stability of Intuitive Surgical’s recurring revenue streams.
Should you buy stock in Intuitive Surgical right now?
Intuitive Surgical may not rank among the top 10 investor picks today, but its fundamentals tell a different story. Consider the growth seen with past Stock Advisor recommendations: investing $1,000 in Netflix in 2004 would now be worth $399,724, while Nvidia’s 2005 recommendation turned $1,000 into $1.37 million by 2026. Though these returns are exceptional, they highlight the potential for well-timed growth investments.
Intuitive Surgical’s stock carries risks, particularly its volatility and premium valuation. However, its established market position and cash-generative business model make it a differentiated bet in the MedTech sector. Investors focused on long-term healthcare innovation exposure may find value in allocating selectively.
