A recent comprehensive review indicates that individuals with Attention Deficit Hyperactivity Disorder (ADHD) face a substantial burden of functional gastrointestinal symptoms—disorders impacting digestive function without necessarily reflecting an identifiable structural pathology.
After screening over 9,000 records and analyzing 23 studies, researchers discovered that ADHD is associated with significantly elevated odds of every gastrointestinal symptom examined, including irritable bowel syndrome, constipation, abdominal pain, diarrhoea, indigestion, and encopresis.
“Nearly every gastrointestinal symptom we investigated appeared to be linked to ADHD,” said co-lead author Sarah Blake, a medical student at Warwick Medical School. “While these symptoms can profoundly impact daily life, they are frequently overlooked when clinical focus remains solely on the neurological condition.”
The analysis revealed a 48% increase in the odds of experiencing gastrointestinal symptoms overall among individuals with ADHD. The association was particularly pronounced for constipation, where the odds nearly doubled, and encopresis—loss of bowel control—where the odds were more than fourfold higher.
Why the link?
The researchers acknowledge several possible explanations, though the study does not establish a direct causal relationship between ADHD and gastrointestinal issues.
Behavioral factors may play a significant role. Emotional dysregulation and heightened stress sensitivity, both common in ADHD, might increase gut sensitivity and contribute to functional abdominal pain. Furthermore, irregular eating habits, missed meals, and binge eating could exacerbate symptoms such as indigestion, bloating, constipation, and abdominal discomfort.
Food-related sensory sensitivities might also contribute. Individuals with ADHD may experience altered sensitivity to food textures and odors, potentially leading to restrictive diets that are low in fiber and subsequently affect bowel habits.
The researchers also highlight altered interoception—the ability to recognize internal bodily signals—as a potential factor. Diminished awareness of the urge to defecate could potentially contribute to constipation or encopresis.
Pharmacological interventions may represent another contributor. Stimulant medications, such as methylphenidate, are known to produce side effects including appetite suppression, abdominal pain, nausea, and constipation. Additionally, sleep disturbances, which are prevalent among those with ADHD, might affect gastrointestinal motility and sensitivity.
The gut-brain connection
The research team also emphasizes the gut microbiome as a potential biological pathway.
Prior studies have identified variations in gut microbial composition between adults with ADHD and neurotypical controls. Other research has linked alterations in the gut microbiome to the manifestation of gastrointestinal symptoms.
One hypothesis suggests that these microbial differences could influence systemic inflammation and the communication pathways between the gut and the brain. Genetic predispositions may also play a contributing role.
“We cannot yet definitively state that ADHD causes these gastrointestinal problems or pinpoint exactly what drives this association, but the pattern is striking enough to warrant close attention,” said Saran Shantikumar, associate clinical professor at Warwick Medical School. “Clinicians managing patients with ADHD should consider inquiring about gastrointestinal symptoms, particularly if they are impacting the patient’s quality of life.”
