NSW Child Protection Review Launched After 4-Year-Old’s Death as Mother Faces Murder Charges]

The New South Wales Department of Communities and Justice received multiple reports concerning a four-year-old child prior to his alleged murder, prompting a government announcement of an independent review into the case handling.

Kate Washington, NSW minister for communities and family, revealed the review on Monday, two days after preschoolers body was discovered at a home on the Central Coast. The child’s mother has been charged with murder and remanded in custody.

Police responding to a call at the Wyong residence found the child’s body with severe arm injuries. The mother, 31, initially approached police on Saturday seeking assistance, authorities said.

District commander Supt Chad Gillies described the scene as confronting even for experienced officers. The Department of Communities and Justice confirmed prior contact with the family and is reviewing all relevant records.

Washington stated the last report about the child was received 18 months ago. She acknowledged case complexities regarding the mother’s substance use and mental health challenges, which will be examined in the review.

The investigation, led by former children’s court president Peter Johnstone, will assess departmental decisions, living arrangements, and protective measures. A report is due by year-end, with findings to be made public where legally permissible.

Washington noted systemic issues, referencing 2024 audit findings that highlighted workforce shortages and gaps in early intervention within the child protection system.

The mother appeared in court on Sunday without bail application and is scheduled to return in September. Both the woman and child are protected under suppression laws.
