In this month’s episode of the Debunked Podcast, co-hosts Arundhati Parmar (MedCity News editor-in-chief) and Samir Batra discussed the CMS ACCESS Model (Advancing Chronic Care with Effective, Scalable Solutions), which launched in July and recently expanded to cover additional chronic conditions.

The ACCESS model ties payments to patient outcomes and promotes tech-enabled preventive care. A recent expansion added heart failure, COPD, substance use disorders, and tobacco cessation to existing tracks for hypertension, diabetes, and behavioral health conditions addressing anxiety and depression. Tech-enabled care in these programs includes remote monitoring, digital behavioral health tools, and virtual care management teams. Batra noted that the ACCESS model outperforms other value-based care efforts, whose monetary formulas for paying digital health companies are often convoluted and frustrating.
AI featured prominently in the discussion. While healthcare organizations see potential for AI to streamline labor-intensive administrative tasks like credentialing, frustration persists regarding the opacity of algorithmic decision-making and how tech companies justify their outputs.
The conversation also touched on Last Week Tonight host John Oliver’s deep dive into UnitedHealth Group’s extensive ecosystem. The segment examined how insurers utilize policies and prior authorization to delay, deny, or complicate care, positioning UnitedHealthcare as a emblem of broader systemic issues within private insurance.
In August, Luigi Mangione changed his plea to guilty in connection with the murder of UnitedHealthcare CEO Brian Thompson, who was killed in December 2024. Mangione faces sentencing on December 18.
