“The major issue A&E departments are currently encountering during extreme heat events is falls among elderly patients,” states Dr Adrian Boyle, an emergency medicine consultant at Addenbrooke’s Hospital in Cambridge. “This summer’s five consecutive heatwaves have led to numerous elderly individuals experiencing dizziness and light-headedness that resulted in severe falls – including a case where an elderly woman fell down a flight of stairs, sustaining multiple broken ribs, a fractured wrist, and lower leg injuries purely due to heat exposure.”
Many elderly patients suffer from conditions requiring diuretics or antihypertensive medications, which while generally beneficial, can dangerously accelerate dehydration symptoms in extreme heat. “We’re seeing frequent episodes of vertigo where heat exposure triggers immediate light-headedness, particularly in medication users. Some have fallen and sustained serious injuries requiring prolonged recovery periods,” Dr Boyle explains.
“Wrist fractures typically require two months of rehabilitation while hip fractures or lower leg injuries may necessitate over six months of recovery. These timelines create substantial burdens on both patients and the NHS,” he adds.
NHS England’s July statistics reveal record-breaking emergency department attendances during heatwaves – with 2.5 million people accessing A&E services. Heat-exacerbated respiratory conditions and cardiovascular emergencies dominated these figures, as extreme temperatures strain already vulnerable cardiovascular systems and compromise respiratory function.
Dr Boyle further highlights additional heat-related health crises: “We’ve treated multiple cases of acute kidney failure due to severe dehydration. Tragically, we’ve also seen young people drowning after swimming in unsupervised rivers seeking relief during heatwaves. Mental health patients face compounded risks when medications impair their ability to perceive rising body temperatures.”
Hospital infrastructure limitations become critical during heat events. “Our facilities lack adequate air conditioning and ventilation systems – hardly surprising when air circulation restrictions prevent infection spread,” notes Dr Zuzanna Sawicka, institutional medicine specialist and clinical director of patient safety. “Medical professionals contend with stifling protective gear during infectious disease visits, compounding heat exposure risks.”
These challenges affect healthcare workers as much as patients. Nurse Jean, from a London hospital, reports persistent fan shortages creating dangerous patient competition: “We have insufficient cooling capacity – patients literally fight over single ceiling fans. The outdated plastic mattresses become unbearably sweaty, adding to staff exhaustion despite thin clothing choices.”
