For decades, preventive healthcare has prioritized extending life expectancy. The next critical focus should be maintaining mobility, independence, and strength to fully enjoy those additional years.
Each era of healthcare has defined a pivotal category. Four decades ago, cardiovascular health dominated the conversation. Two decades later, metabolic health took center stage. Recently, optimizing sleep, hormones, and longevity has been the emphasis. While these trends share a common trajectory, bone health is poised to join their ranks.
Bone health, historically overlooked despite its profound impact, is now entering a transformative phase. Over 60 million Americans over 50 either have osteoporosis or osteopenia, yet these conditions remain underdiagnosed and undertreated. Globally, fragility fractures occur at a staggering 37 million annually—one every second. Half of women and one in four men over 50 will experience an osteoporotic fracture in their lifetime. Despite this burden, bone health has never been integrated into everyday preventive care like cardiovascular or metabolic health.
Bone health represents the largest untapped preventive healthcare opportunity. Aging demographics, with the U.S. population over 65 projected to reach 83.7 million by 2050, underscore this shift. Menopause accelerates bone density loss—women can lose up to 20% within five to seven years post-menisopause. Meanwhile, the surge in GLP-1 medications for weight loss poses risks of bone density and muscle mass decline, potentially increasing fracture risks. The longevity movement, emphasizing healthspan over lifespan, further highlights skeletal strength as essential for maintaining independence.

Artificial intelligence is democratizing diagnostics by extracting clinically relevant data from routine imaging and electronic records. Millions of CT scans and X-rays performed for unrelated conditions already contain overlooked bone density information. AI-driven opportunistic screening tools are now capable of identifying osteoporosis risk, revolutionizing how bone health is assessed. This shift transforms bone health from a diagnostic specialty to a longitudinal care metric, mirroring advancements in cardiovascular and metabolic health.
The preventive health stack—encompassing heart, brain, metabolism, sleep, and mental health—has undergone a revolution, but bone health has lagged. Key barriers include measurement challenges, silent disease progression, inadequate tracking, limited treatments, and low consumer awareness. Emerging solutions like AI-enabled diagnostics, heightened menopause awareness, longevity focus, and the urgent need to monitor GLP-1 users are dismantling these obstacles.

The future of bone health centers on mobility, not merely osteoporosis. Achieving this requires three interconnected shifts: early intervention through accessible AI screening, multimodal treatment integrating exercise, nutrition, fall prevention, and drug therapies, and personalized treatment optimization based on longitudinal data. These advancements will make bone health as manageable as blood pressure or cholesterol today, ensuring fractures become rare rather than an accepted aspect of aging.
Transformative healthcare evolves through scientific breakthroughs, technological accessibility, consumer engagement, and economic necessity. Bone health has reached this inflection point. The question is no longer whether it will become a major preventive category—but who will lead its definition. Our vision is a future where fractures are uncommon and millions more live active, mobile, and strong lives.
