War has always been a brutal engine of medical innovation, operating as a feedback loop where civilian breakthroughs are adapted for the battlefield and battlefield necessity drives advances back into civilian care. To identify the most influential developments over the past five years, we consulted several experts in the field.

John Quinn, an adjunct lecturer at the College of Remote and Offshore Medicine Foundation and a medical volunteer in Ukraine, emphasizes that the most critical combat‑medical innovations are not gadgets but new ways to distribute risk, skill, and authority. First, “blood‑forward” resuscitation means moving whole blood, plasma, calcium, warming agents, tranexamic acid, and transfusion decisions much closer to the point of injury. Second, a training and staffing revolution is essential: medics, nurses, combat medics, and generalist physicians now need damage‑control resuscitation, ultrasound interpretation, prolonged casualty care, and selected surgical procedural skills because evacuation can be delayed or denied and medical personnel are deliberately targeted. Third, AI‑enabled decision support—from smartphone applications and ultrasound guidance to cardiac monitoring, triage algorithms, documentation tools, and evacuation prioritization—will play a central role. A fourth development is advanced endovascular and damage‑control thinking, including partial resuscitative endovascular balloon occlusion of the aorta (REBOA) as a bridge for non‑compressible hemorrhage. The next leap will integrate these lessons into autonomous casualty movement, blood and supply delivery, sensor‑driven mass‑casualty triage, and AI‑guided treatment pathways that help mid‑level and even non‑clinical responders decide who needs care first and what to do.

Kimberly Elenberg, Principal Project Scientist at Auton Lab, Robotics Institute, Carnegie Mellon University, highlights the tangible technologies that have emerged in the last half‑decade: low‑titer O whole blood, walking blood banks, FDA‑cleared freeze‑dried plasma now reaching joint forces, shelf‑stable substitutes, and hemorrhage‑control measures and surgical airway techniques that have been pushed down the skill chain to a broader range of providers.
